# Errors

Failed Phone Match v3 requests return `status: "failed"` and a message describing the failure.

| HTTP status | Meaning | Common causes |
| --- | --- | --- |
| `400` | Request timeout or upstream access failure | The request could not be completed in time. |
| `401` | Authentication failed | Missing or invalid credentials, timestamp, or signature. |
| `403` | Forbidden | The merchant or channel is not enabled for Phone Match. |
| `422` | Validation failed | Missing or invalid `transaction_id`, `nik`, `msisdn`, or `channel`; unsupported operator. |
| `500` | Processing failed | The operator request could not be processed. |
| `503` | Maintenance | The service is temporarily unavailable. |

Validation messages can include `transaction_id is required`, `msisdn is required`, `channel is required`, length or format errors, and an unsupported-operator error. Correct the request before retrying a `422` response.
